A squeeze of fresh lemon (approximately 30ml of juice) contains about 8 calories and 3g of carbohydrates. Whether this breaks a fast depends on your fasting goals: for caloric fasting, technically yes. For ketosis maintenance, 3g of carbs has negligible impact on blood glucose or ketosis.
Research shows that small amounts of lemon juice do not produce a meaningful insulin response in healthy individuals. The citric acid and polyphenols in lemon may actually support digestive function without metabolic disruption.
Lemon water will not knock you out of ketosis. For most intermittent fasting protocols on keto, it is entirely acceptable. For strict caloric fasting protocols (OMAD, 72-hour fasts), purists may prefer plain water.
